New York State Immunization Requirements

For School Entrance/Attendance for Kindergarten:

• DPT 4 - 5 doses
• Polio 3 - 4 doses
• MMR 2 doses 
• Hepatitis B 3 doses
• Varicella (chicken pox) - 2 doses

In addition, children entering 6th grade need:

• Tdap - 1 dose  (before reaching 6th grade or once child turns 11)
• Varicella - 2 doses
• Polio - 4 doses
